The PHMC Health Network recently received an award at five of its Federally Qualified Health Centers (FQHCs) for implementing a program to help patients measure and manage their blood pressure.
Target: BP is a national initiative of the American Heart Association and the American Medical Association aimed at supporting health care organizations in improving blood pressure control rates through an evidence-based quality improvement strategy. Earlier this year, nurses at five of our FQHCs – PHMC Care Clinic, PHMC Health Connection, Mary Howard Health Center, Rising Sun Health Center and Congreso Health Center – began their participation in the program by educating patients on healthy eating and exercise habits, creating treatment plans and providing a tool for them to measure their blood pressure daily at home.
As a result of their efforts, the FQHCs were awarded the Target: BP Silver Award, which recognizes practices that have demonstrated a commitment to improving blood pressure control through measurement accuracy. The program has recently expanded to include staff and patients at the PHMC Health Center on Cedar.
“One of the most important aspects of this program has been empowering our patients,” said Chanel Hart, PHMC’s Director of Nursing. “By providing nutrition education and the self-measurement devices, we’re giving them the knowledge and tools to monitor and improve their health independently.”
