PHMC Rising Sun Health Center provides specialized primary care for each patient’s needs. The health center is within the One & Olney Shopping Center. Our team includes bilingual health professionals and offers interpretive services. PHMC Rising Sun Health Center is a Patient-Centered Medical Home.

Our health care services include:

• Primary care for adults, children and families
• Vaccinations
• Family planning, including contraceptive counseling (IUD and Nexplanon placement) and education
• Women’s health services
• Behavioral health evaluations and consultations
• Testing, counseling, and treatment for HIV and Hepatitis C
• Medication-Assisted Treatment and peer recovery services
• Health and nutrition counseling
• Management of chronic (long-term) health conditions (diabetes, high blood pressure, asthma, etc.)
• Medical case management and help with referrals
• Social services support (SNAP, housing, insurance, utilities, legal, etc.)

We accept most insurance plans. A sliding fee scale, based on income, is available for uninsured and underinsured patients. We see all patients regardless of their ability to pay.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). This health center also receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
