PHMC Health Connection provides primary care services to children, teens and adults. Our team includes bilingual staff and offers interpretive services. PHMC Health Connection is a Patient-Centered Medical Home.
Our health care services include:
- Primary care for adults, children and families
- Vaccinations
- Family planning, including contraceptive counseling (IUD and Nexplanon placement) and education
- Women’s health services
- Behavioral health evaluations and consultations
- Testing, counseling, and treatment for HIV and Hepatitis C
- Medication-Assisted Treatment and peer recovery services
- Health and nutrition counseling
- Management of chronic health conditions (diabetes, hypertension, asthma, etc.)
- Medical case management and referral navigation
- Social services assistance (SNAP, housing, insurance, utilities, legal, etc.)
Most insurance plans are accepted. A sliding fee scale is available for uninsured and underinsured patients. We see all patients regardless of their ability to pay.

Regional Rail Lines – Temple University Station.
47 Bus – One block from either 8th Street and Norris Street or 8th and Berks Street stops.
23 Bus – Two blocks from either 11th Street and Norris or 11th Street and Berks Street stops.
This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). This health center also receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
