PHMC Health’s Federally Qualified Health Centers (FQHCs) recently received an award for implementing a program to help patients manage their cholesterol levels.
Check. Change. Control. Cholesterol™ is an initiative of the American Heart Association aimed at improving the screening and management of cholesterol in order to reduce the number of Americans who have heart attacks and strokes. Our FQHCs – PHMC Health Callowhill, PHMC Health Center on Cedar, PHMC Health Connection, Rising Sun Health Center and Congreso Health Center – participated in the program by educating patients on healthy lifestyle habits and creating treatment plans to help them lower their cholesterol levels.
As a result of their efforts, the FQHCs were awarded the Check. Change. Control. Cholesterol Gold™ Award, which recognizes practices that have 70% or more of their adult patients at risk of atherosclerotic cardiovascular disease appropriately managed with prescription medications.
“One of the keys to our program’s success is helping patients maintain ownership during the process,” said Dr. Pheobe Askie, PHMC Health’s Medical Director. “By giving them the information they need and partnering with them in their treatment, we’re equipping them to make changes and improve their health independently.”
