PHMC Medical Respite on Cedar now offers post-acute medical care for individuals experiencing homelessness who are discharged from a hospital and require additional medical care to recover from a physical illness, surgery or injury. Located in the PHMC Public Health Campus on Cedar, the respite offers high-quality medical care, linkage to health and social services, and comfortable space for rest and recovery.
PHMC’s Medical Respite on Cedar and Medical Respite at Serenity Court are the only programs of their kind in Philadelphia. People experiencing homelessness have higher hospital inpatient and emergency department readmission rates than the general population. Medical respite programs offer supportive recovery space that shelters are not generally equipped to provide while reducing hospitalization and health system costs by coordinating primary and behavioral health services for patients.
PHMC Medical Respite is part of an expanding range of services offered at the PHMC Public Health Campus on Cedar. The campus currently provides primary care services for families at the PHMC Health Center on Cedar and recently launched an inpatient hospice unit operated by PHMC subsidiary The Visiting Nurse Association of Greater Philadelphia. Additional services include an Emergency Department, inpatient hospital care and psychiatric care operated by the Hospital of the University of Pennsylvania, and additional programming offered by Children’s Hospital of Philadelphia.
